Adapta promotes the physical, emotional, and spiritual well-being of individuals with brain injuries, mental health conditions, and substance use disorders, in addition to a variety of health concerns. Our person-centered, family feel allows us to build relationships with the individuals we serve.

Adapta operates four locations, each offering a unique variation on care. Folwell Flats is home to 18 individuals, providing personalized care in a dorm-like environment. Manor House and Judd House are community-based residential homes with their own charm, fostering a sense of belonging. The 55th Street Apartments is home to individuals aged 55 and older, offering comfortable living spaces that promote community and healthy living.

At Adapta, we provide personalized programs tailored to the unique needs and desires of each individual. As a leader in community integration, our robust Life Enrichment Program enhances the quality of life for our residents. Our dedicated staff offers a variety of services, including nursing care, recreational activities, transportation to appointments and events, 24-hour supervision, medication management, meals, shopping assistance, and money management support. Additionally, we collaborate with a variety of community partners to offer an array of contracted services, ensuring our residents receive the support they need to thrive.
